Mohammed T. Rezai, DO

Dr. Rezai is a double board-certified physician, hailing from the Southern California region, specifically Northridge, CA, where he spent his formative years. He pursued his undergraduate education at the University of California, Los Angeles (UCLA) before embarking on his medical journey at Touro University located in Vallejo, CA.

Subsequently, he undertook a rigorous rural family medicine residency program at the Shasta Community Health Center in Redding, CA, which is affiliated with the University of California, Davis (UC-Davis). Dr. Rezai holds the esteemed status of being a diplomate of both the American Board of Family Medicine and the American Board of Preventive Medicine with a specialization in Addiction Medicine.

Dr. Rezai has garnered accolades from respected medical associations, including recognition from the Ventura County Medical Association and commendation from the California State Legislature for his exceptional contributions to the community during the challenging COVID-19 pandemic. When not engaged at Zai Health, Dr. Rezai dedicates his time to community service, demonstrating his commitment through active affiliations with multiple domestic violence shelters situated within the expansive Ventura County region.

Matthew “Matt” Butler, PA-C

o Matt Butler, PA-C, is a highly-qualified board-certified Physician Assistant with a remarkable record of more than six years in the specialized field of treating patients grappling with opioid use disorder. Matt's educational background includes the attainment of his Master of Medical Sciences degree from Salus University, located in Pennsylvania. Additionally, he holds the distinguished status of being a National Health Service Corp Scholar, demonstrating his dedication to the advancement of healthcare services. Matt is proficient in both English and Spanish, broadening his capacity to serve diverse patient populations.

Throughout his career, Matt has witnessed the transformative impact of medication-assisted treatment on the lives of numerous patients. This experience has deepened his commitment and passion for guiding individuals towards recovery, enabling them to rebuild their lives on a solid foundation.

Jared Fife, PA-C

Jared Fife, PA-C, is a highly proficient and compassionate board-certified Physician Assistant specializing in the treatment of opiate use disorder. He holds a distinguished educational background, having earned his Master's degree from Arcadia University. With a career spanning back to 2015, he has garnered extensive experience in family practice and addiction medicine.

Jared is honored to serve as a medical provider at Zai Health, where he plays a pivotal role in ensuring that patients have access to addiction care services through telemedicine, thus advancing the reach and quality of care.
